Work through these in order. Most failures come from the first few.
Checklist
Too small: print at least 2 cm wide for close range, larger for distance. Low contrast: use dark modules on a light background. Inverted colors: switch back to dark on light. No quiet zone: keep a clear border of at least four modules. Too dense: a very long URL makes a complex pattern; shorten the link or use a dynamic code. Logo too big: shrink it or raise error correction.
Physical problems
Glossy surfaces reflect light, curved surfaces distort the pattern, and low-resolution images blur modules. Export SVG or a high-resolution PNG and never stretch a small image.
On-screen codes
Screens can cause moiré when scanned by another screen. Increase brightness, hold steady, and make the code larger on the page.
If a code works on one phone but not another, it is borderline: increase its size or contrast.
